1) What other radiopharmaceutical could have been used?
111-indium octreotide could have been used. Although octreotide is a more expensive radiopharmaceutical, it is more sensitive for detecting carcinoid tumours than MIBG. Having said that, very occasionally, a carcinoid tumour will take up MIBG but not octreotide.2) Why do carcinoids take up the tracer?
Many carcinoids contain neurosecretory granules and take up MIBG into these.3) Is there any extrahepatic disease
There are multiple hepatic metastases but no evidence of extrahepatic uptake.
